Magnus Health and Student Physical Exams

The Magnus Health Portal will open for all families on June 1 for the 2020-21 school year. All Trinity students are required to complete a Magnus Health Profile, which will include permissions to participate in activities and a space to upload your most recent physical exam and medical history (see below). Parents will receive an email on June 1 with detailed instructions about how to access and update their Magnus Health profile.

 

All returning students are required to have a physical exam completed and dated AFTER May 1, 2020 by a physician and uploaded to the Magnus Health profile before participating in any on-campus Trinity activity. Please bring BOTH the Trinity Student Physical Exam (PDF) form and the Student Medical History (PDF) with you to the exam.

 

As we continue to receive guidance from public health officials and the Virginia Department of Health around COVID-19, we may be adjusting timelines and requirements relating to completion of portions of the Magnus Health Profile. We advise parents to first reach out to their primary care physician for guidance on the appropriate time to complete their student's physical exam and other required forms.
